As we prepare for graduation, we want to make sure everything is accurate and that students have the support they need for their next steps. This form will allow you to:

Diploma Name – Confirm the student’s name exactly as it should appear on the diploma.
Next Steps After High School – Share future plans (college, trade school, military, work, etc.) so counselors can connect students with resources.
Application & Counseling Support – Request help from school counselors with college, or financial aid applications.
Contact Updates – Provide any updated addresses, phone numbers, or emails, This helps ensure you received all important graduation updates
